Lymph Node Ratio for Nodal Staging in Colorectal Cancer - a Promising, but Premature Tool

چکیده

10-14 regional lymph nodes, nodal staging in stage III colorectal cancer, which is solely based on the number of metastatic lymph nodes, would be inaccurate in certain circumstances. Recently, the metastatic lymph-node ratio has been suggested as an alternative tool for nodal staging in stage III colorectal cancer [2-4]. Also, after preoperative chemoradiation therapy in rectal cancer patients, the lymph-node ratio has reportedly been able to provide better stratification of patients’ prognoses than ypN staging [5]. A recent series of publications support the lymph-node ratio’s being a successful substitute for the current nodal staging in overcoming the limitation of the numeric counting system in cases of colorectal cancer. However, there are still several problems in the studies regarding the lymphnode ratio. The biggest hurdle in nodal staging with the lymph-node ratio may be that there is no consensus on a point of reference for stratification. Even though the lymph-node ratio has shown very nice stratification of patients’ prognoses in cases of stage III colorectal cancer, not all studies adapted the same point of reference. Standardization of the point of reference would be an essential step toward use of the lymph-node ratio in staging. There is no doubt that precise lymph-node staging would be an essential step toward providing accurate prognoses for colorectal cancer patients and selecting candidates for adjuvant therapy. Several studies have demonstrated the possibility of using the lymph-node ratio as a tool for more accurate nodal staging in colorectal cancer. However, many obstacles remain to be overcome before the acceptance of the lymph-node ratio in our daily practice.

Molecular staging estimates occult tumor burden in colorectal cancer.

Tumor cells in regional lymph nodes are a key prognostic marker of survival and predictive marker of response to adjuvant chemotherapy in colorectal cancer. However, clinicopathologic techniques to detect lymph node metastases remain imperfect, and approximately 30% of patients with lymph nodes negative by histology (pN0) develop recurrent disease, reflecting occult metastases that escape detec...

نقش پاراآئورتیک لنفادنکتومی در مرحله یک سرطان تخمدان

Background: Surgical staging is the standard treatment of ovarian cancer. Pelvic and para-aortic lymphadenectomy is the important part of the surgery. The aim of this study was to evaluate the effect of para aortic lymph node dissection in early stage of patients with ovarian cancer. Methods: This descriptive cross-sectional cohort study was performed on all stage I of ovarian cancer pati...

Sentinel Node Mapping in Non-small Cell Lung Cancer Using an Intraoperative Radiotracer Technique

Objective(s): Lymph node metastases are the most significant prognostic factor in localized non-small cell lung cancer (NSCLC). Identification of the first nodal drainage site (sentinel node) may improve detection of metastatic nodes. Extended surgeries, such as lobectomy or pneumonectomy with lymph node dissection, are among the therapeutic options of higher acceptab...

[Sentinel lymph node mapping in patients with colorectal cancer].

UNLABELLED The colorectal cancer is the most frequent and clinically significant tumor of the gastrointestinal tract. For more than 100 years, radical lymphadenectomy has been the diagnostic and therapeutic gold standard for the management of metastatic nodal disease. More than 50% of patients are initially present with I and II stage of the disease. About 15% to 30% of these patients develop r...

Lymphatic mapping and sentinel node analysis to optimize laparoscopic resection and staging of colorectal cancer: an update.

BACKGROUND Laparoscopic colectomy for colorectal cancer (CRC) has been criticized because of the potential for inadequate nodal dissection and incomplete staging. Lymphatic mapping (LM) and sentinel lymph node (SLN) analysis can improve the accuracy of staging in open colectomy, but its utility during laparoscopic colectomy is unknown. METHODS Between 1996 and 2002, 30 patients with clinicall...
